The Case Counsellor is responsible for providing telephonic support and follow-ups to eligible newly initiated and are-initiated patients on ART with the aim of retaining them in care and treatment until they are virally supressed and placed on the decanting programme . They will also be responsible to provide education and information around treatment and care of HIV and or TB. The incumbent should also be tracing Pre Art, and defaulters to ring them back into care
Technical Work Responsibilities
•Coordinate with healthcare facilities to locate identified HIV- positive clients /cases requiring linkage back to care
•Register and maintain all eligible newly initiated patients on ART
•Coordinate referrals to clinic and community-based HIV care and treatment services, and advocate for accessibility to these services
•Conduct follow-up visits to deliver case management support to assigned patients as per case management protocol
•Liaise with case facilitator on the clients who are lost to follow up and needs to be telephonically traced .
•Liaise with clinical teams on any clinical guidance that needs to be followed up
•Ensure appropriate recording and reporting of all data obtained
•HIV/AIDS Counselling and Testing Certificate
•Relationship Counselling Certificate is this a qualification?
•Trauma Counselling Certificate
•Given that this person will be dealing with clients, client centric training should be added, alternatively some training in customer care especially in this field
•2 years’ HIV testing and/or counselling
•2 years’ HIV and TB community education and mobilisation
•Customer Support Service
•SA Citizen or valid work permit to work in South Africa, given that this is a low profile position, we should limit to SA citizens only
•Basic computer skills
•Excellent social kills, team player, performance oriented, and self-motivated
•Excellent communication skills, listening skills, knowledge retention and the ability to establish effective, trusting relationships
•Demonstrable empathy, compassion and an understanding of the cultural and socioeconomic backgrounds in the community
